Referrals
Our referral department provides referrals for specialist consultations, diagnostic studies and therapy visits, as well as insurance preauthorizations as required.
The office is open Monday-Friday and referral personnel can be reached at (503) 370-4225.
All requests must be approved by your primary care physician before scheduling or authorization takes place.
